Resident Group Care Opportunities

Please see the Schedule of all current and upcoming groups (previous screen, ccrmc.wikispaces.org.Group+Visits) Some of the groups are continuous; others are discrete series of 2 to 12 sessions.
Some of the group providers are comfortable hosting a resident who wants to observe one or two groups. Some are also comfortable precepting residents who want to make more of a commitment and co-facilitate a series. Some of the group providers do not feel comfortable yet with hosting/teaching residents. All the ones listed below welcome residents.

If you co-facilitate a series, you may be able to apply all group participants toward your required 1650 continuity clinic visits requirement. Please discuss this with a Lead Preceptor first, to make sure the visits will qualify and to plan for the necessary record-keeping.

With the group provider's OK, you can schedule yourself for any group that doesn't conflict with your other clinical responsibilities: e.g., elective or Clinics rotation time that is not scheduled for FPC. For Clinics rotation scheduling, you must first get approval from Anni Hansen, because there are certain clinics that cannot be canceled for this purpose.
We do not have permission for you to use other outpatient time (eg peds or gyn clinic time).

Below are descriptions of groups submitted by group providers who have confirmed that they welcome resident participation.

Farey RHC Prenatal group (applies to Shah, Harvey, and Siegel also), RHC
I'm open to residents coming for one or more prenatal sessions, and think the optimum is to participate in a series of 10-11. I think it's about 1/2 hour prep for each session, previewing charts, and more to prep facilitating topics. I would have residents both doing belly checks and facilitating discussions. I think they should understand and know something in advance about group prenatal care, understand what they are committing to, which if a series, is the whole series. I think they should have basic facilitation skills, and some facility with Spanish. If interested in co-facilitating a series, contact Krista Farey at least 5-6 months in advance. Second half of second year and first half of third year are optimal times to participate.

Glatt RHC Diabetes Group
Richmond Health Center Diabetes Group was modeled after the Centering model. Co-facilitated by Dietician/CDE, MD, Promotora or Health Conductor. Participants are empowered to take an active role in their diabetes self-management. The group setting provides social support and promotes positive behavior change. Participants learn about Diabetes 101 basics (A,B,C's), set and review personal action plans, meal planning, and how to prevent complications of diabetes. Teaching methods are interactive with opportuniities for direct learning using real food and food models, dry insulin injection practice, and group glucose monitoring. Patients take their own vital signs, including use of self-inflating BP cuffs, and help chart and review their own progress note for the visit. The MD conducts medical pull-outs to optimize blood pressure, reconcile medications, and meet diabetes care metrics. Charting is done collaboratively among facilitators. Residents may participate as co-facilitators and assist with medical pull-outs.
It meets the 1st and 3rd Thursday morning for three consecutive months. Moderate Spanish fluency is necessary for Spanish groups.

Hennigan, Pain and Wellness, Pittsburg, Martinez, and Richmond
Residents can participate in the group in episodic or long term ways. They can contact me by email if it is outside a regular rotation where they are already scheduled to come. We try to orient residents their first day and have an orientation packet. There is nothing special they need to do to prepare, we just give more responsibility once they've acclimated.
Some of the groups are staffed by a nurse practitioner rather than a family physician, so those groups could not count toward your 1650 visits despite your participating a series of times.

Kaji Lao (Khmu and Mien)
Dr. Kaji and the interpreter address a general health issue with the group from 9-9:30. After that, they see individual patients while the rest of the group continues the discussion and socializes. These are long-term, stable patient/community groups.

Saffier buprenorphine group
Monday ams: 8 - 12. Contact me beforehand via email or by pager. Best to attend several groups over weeks to months, resident's schedule permitting. Prior reading, discussion prior to group desirable but not required.

Hollander (RD, CDE) Diabetes and Preventing Diabetes Groups, RHCArinna Hollander, Registered Dietician and Certified Diabetic Educator, has been part of the dynamic Chronic DIsease Management team at RHC for years. Arinna offers a two-part classes for diabetics, as well as aPreventing Diabetes class for people with any DM risk factors. These classes are in English.
Patients in these groups would not apply to your 1650 patient visit requirement, because the groups are not done with an attending family physician.
